     NY State Immunization requirements for all students entering 6th grade is a TDAP Vaccine and a 2nd varicella vaccine.

     

    -For Tdap-All students who are 11 years old at the start of 6th grade must show proof of this vaccine prior  to 6th grade admission. If your child  has not turned 11 by the start of 6th grade, your doctor may choose to wait until their birthday to administer the vaccine. These children must have a medical note  from the doctor stating that the doctor is waiting until their 11th birthday.

    -For varicella- proof of immunization and or a health care provider's signed medical record indicating the student had the disease is also acceptable.

    -New York State mandates that no child can be admitted into 6th grade without this documentation.

    All 7th grade students are required to have a meningitis vaccine to start school and a recent physical on file.

    All students entering 7th grade and all students trying out for a sport during the school year  must have a recent physical. The physical exam is valid for 365 day. In an effort to help you comply with these requirements see the forms attached to this site.

    • A form -physical for doctor
    • B form- re-evaluation form filled out by parent/student for each sport,to be handed in no sooner than one month prior to the start of each sport season

    If you did not see the school doctor, please make an appointment with your doctor to get a physical.

    Any medication to be administered at school ( prescription or over the counter) must have a prescription from a medical doctor. The form for the doctor can also be found on this web site. It will also need to be signed by the parent.  You and your doctor can choose between the child self carrying the medication or the nurse holding and administering the medication.

    The emergency card is available on this web site also. Please note that it is to be folded so part will be up-side down.

    A child who is sick will not be able to perform well in school and is likely to spread the illness to other children and staff.  Our school policy states that you should not send your child to school if he/she has: fever in the past 24 hours,vomiting in the past 24 hours, diarrhea in the past 24 hours, chills, strep throat and pink eye( must have been taking an antibiotic for at least 24 hours before returning to school), bad cold with a very runny nose or bad cough, ,and head lice( must be nit free).

    Please notify the nurse if you child has a communicable disease or other medical condition or a change in a known medical condition.
